Online Application Link

Financial assistance applications are widely available throughout the hospital and clinics or can be completed online at The application for financial assistance is good for one year and must be completed and refiled annually. Completed applications must be received within two weeks of the initial request and KHC staff will respond within two weeks of receiving the completed application to let you know if you qualify along with the discount percentage.

Once a financial assistance discount determination has been made for those who qualify and the discount has been applied, the patient is responsible to work with KHC to make arrangements to pay the remaining balance on the bill(s). In the event that a remaining balance due after a financial assistance discount has been applied does not remain in good standing, KHC reserves the right to suspend or revoke the financial assistance discount. In addition, an outside collection agency may be engaged to deal with the matter.
