The Clinic Triage Nurse is an RN that holds a current license in the State of Iowa and will facilitate the caller’s access to the appropriate level of care by assessing the patient’s needs over the phone. Provide professional nursing skills in a defined outpatient clinic setting. Apply professional nursing skills in the provision of preventive health maintenance and treatment of ill and injured patients using the four aspects of the nursing process: assessment, planning, implementation and evaluation. Screen and respond to patient phone calls, directing to the Primary Care Provider as necessary. Assist healthcare provider with clerical/clinical functions for patients requiring chronic disease coordination. Promote wellness and informed decisions by providing health information and prevention concepts. Act as a patient advocate by assisting with the resolution of problems, communication of patient needs and concerns to the appropriate resources. Initiates written and verbal follow-up communication for each encounter. Demonstrate ability to recognize and assess emergency and non-emergency needs of patients. Report results and pertinent information to patients and health care team members. Function within the limits and scope of RN license. Adhere to HIPAA and maintains patient confidentiality at all times.
• Graduate of an accredited school of Nursing
• Current RN license in the State of Iowa
• Current BLS and Mandatory Reporter certifications
• Two years of experience in a clinic/physician office preferred
• Proficient use of computer
