Clinic Wellness RN – PRN Clinic

The Wellness RN assists patients in preventative health maintenance with an increased emphasis on improving the patients’ overall wellness and sense of well-being. The position will work closely with our patients and their insurers in promoting wellness and helping individuals realize their personal best. The Wellness RN is responsible for developing an effective and efficient interpersonal relationship with patients and assisting the patient through the process of actively working towards better health by providing support, encouragement and education. The Wellness RN will conduct outreach to patients who need care services utilizing care guidelines and standing orders to schedule recommended services for disease management patients. Is an integral member of the clinic health care team who works to ensure safety, best practice, and high quality standards of care are maintained. Provides oversight of the disease registry database including assuring database is maintained and kept up to date. Creating patient, physician, and clinic level quality performance reports. Provides proactive outreach to patients including identifying patients overdue for visits, labs, or referrals and arranging for follow-up services as appropriate. Identifying patients not meeting clinical goals, such as BP control or glucose control, and arranging for follow-up services by protocol or as appropriate. Provides oversight of pre-visit chart review of patients including providing disease registry visit manager form prior to patient office visits so the appropriate clinical staff can identify and address any gaps in care during the office visit. Assists with Medicare programs as well as other incentive programs. Anticipates needs and assists patients while impacting relationships in a positive manner and maintaining patient care as priority.
Registered Nurse, BSN preferred, with certification from an accredited professional health coach training program preferred. Knowledge of specific disease and lifestyle related topics such as smoking cessation, weight management, nutrition, stress reduction and chronic conditions. Certification/Licensure in the State of Iowa if applicable. Previous acute or clinic experience. Current BCLS verification.
