Community Health Worker

Full Time position working 36 hours a week. 

Essential Functions

  • Work closely with medical provider to help ensure that patients have comprehensive and coordinated care.
  • Collaborate with other professionals to assess client needs and social determinates of health and/or plan treatments.
  • Act as patient advocate and liaison between the patient/family and community service agencies.
  • Responsible for establishing trusting relationships with patients and their families while providing general support and encouragement.
  • Advocate for clients or patients to resolve crises.
  • Intervene in crisis situations to assist clients.
  • Refer patient, client, or family to community resources to assist in recovery from mental or physical illness and to provide access to services such as financial assistance, legal aid, housing, job placement or education.
  • Interview clients to gather information about their backgrounds, needs, or progress.
  • Identify environmental impediments to client or patient progress through interviews and review of patient records.
  • Collect information from patients.

  • Evaluate potential problems in home or work environments of clients.
  • Work independently with minimal supervision.

Job Requirements

  • Bachelor’s degree in public health or social work is preferred.
  • 2-3 years of experience in areas related to public health, social work or patient advocacy.  Experience in a medical environment a plus.
  • Must possess a cheerful and extroverted personality that reflects a positive attitude toward the hospital and community.
  • Appearance is always neat and well groomed.
  • Work effectively with others.
  • Must demonstrate proficiency with MS Office and Excel and be able to interpret data into meaningful information.
  • Must possess strong organizational skills, be detail oriented, and able to interact and build relationships both internal and external.
  • Must have Mandatory Reporter training.
  • BLS Certification.
  • Other duties as assigned.
