IRIS, Iowa’s Immunization Registry Information System, is a no-cost internet-based service offered by the Iowa Department of Public Health. It is a secure and confidential registry available to all Iowans.

IRIS safely stores immunization information from multiple sources electronically into one centralized system.

IRIS helps parents and individuals keep track of their immunization status, even if they have received vaccines from more than one health care provider. Information available through IRIS comes from a variety of sources, including private health care providers, pharmacies, public health clinics, Medicaid claim administrators, and others.

Online access is an excellent time saver, eliminating the need to request and then wait for immunization records from your health care providers.

Providers authorized to use IRIS can see what immunizations your child already has had, even if they were given in another city or county. So, when you come in for shots, your child gets only those that they need.

This means your child won’t be under-vaccinated and more susceptible to diseases. Your child won’t be over-vaccinated either, so they don’t have to go through any more discomfort than is necessary.

IRIS can print out an immunization record you can use to get into school or daycare.

Using IRIS can also remind you to bring your child in for shots that are due.

Who submits records to IRIS?

Since 2001, participating health care providers have had the ability to update patient records in IRIS. IRIS is used by more than 1,700 health care provider organizations and includes more than 2.6 million patient records.

How can I access my family’s records?

Adults, parents, and guardians can access their immunization records or their child’s record by going online to Public Immunization Record Access. If you don’t have Internet access, you can call the IRIS Help Desk at 800-374-3958.

What personal information will be needed to access the records?

For each individual or member of your family you will need:

  • First name
  • Last name
  • Date of birth
  • Social Security Number or Medicaid ID

Once you enter this information, the system will look for records that match exactly all the information entered.

It will display a list of vaccines the person has received and those the person currently needs.
