Community Health
Navigator
Connecting patients with local health care resources
Free resource assistance
Community Health Navigators are trained to address a host of obstacles to help individuals live healthy lives. They have the tools to connect individuals with community resources that might help them:
- Apply for insurance
- Find a health care provider
- Find transportation to appointments
- Get needed medication or supplies
- Get connections for housing, food, utilities, employment, life skills, or other needs
The Community Health Navigator position at KHC is currently funded through a three-year grant by HRSA (Health Resources and Services Administration) in partnership with MercyOne Newton and Dallas County Hospital. The purpose of the grant is to identify and address social determinants of health in rural communities through networking with local resources to help address social needs, including housing, food availability, utility costs, transportation, medication affordability, social isolation, and health literacy.
The Community Health Navigator services at KHC are free and available to all KHC patients. Individuals can refer themselves or be referred by their health care provider.
