At Knoxville Hospital & Clinics, we are proud to offer Live Well - Prevent Diabetes: Medicare Edition...
an evidence-based pilot program designed to help you reduce your risk of developing Type 2 Diabetes.
This is a free pilot program for eligible participants (see criteria below). To support the sustainability of future diabetes prevention services throughout Iowa, our partner, The HUB (a CDC-recognized Umbrella Hub Organization) will handle both registration and billing your Medicare insurance. The HUB is working towards becoming a CMS-approved biller for KHC, as well as other organizations that will offer future diabetes prevention programs.
How the Live Well - Prevent Diabetes: Medicare Edition program works.
Step 1: Your KHC provider will refer you to the program. To be eligible, all of the following criteria must be met:
1.) Have a body mass index of 25 or higher
2.) Not be previously diagnosed with Types 1 or 2 Diabetes
3.) Not be pregnant
4.) Have Medicare traditional insurance (red/white/blue; not Medicare Advantage) as your primary insurance
5.) Meet one (1) of these criteria:
a. In the past 12 months, have an:
– A1C of 5.7% to 6.4%, OR
– Fasting blood glucose of 100 to 125 mg/dL, OR
b. Be previously diagnosed with gestational diabetes
Step 2: The HUB will contact you to complete registration and confirm details on your Medicare (traditional) insurance.
Step 3: You begin the program on Tuesday, February 3, at 10 a.m., at KHC.
Your cost is $0. You will incur no out-of-pocket fees, copayments, or deductibles.
Together, we will cover topics including healthy eating, physical activity, weight management, stress management, and staying motivated - everything you need to make lasting lifestyle changes.
About the Live Well - Prevent Diabetes: Medicare Edition program
The Live Well – Prevent Diabetes: Medicare Edition pilot is a year-long program, running from February through January 2027. Classes will be held on Tuesdays from 10:00-11:00 a.m. in the KHC Learning Center.

Classes will be a small group, interactive setting where participation is voluntary.
In-person attendance is preferred, however, a Zoom link option is available.

Laurynn is a Registered and Licensed Dietitian and Certified Diabetes Care and Education Specialist. As a nutrition specialist, Laurynn is passionate about inspiring people to eat nutritious and delicious food to live a healthy and happy life.
Dietitians are the food and nutrition experts. Laurynn can assist you on your health and wellness journey for a variety of reasons – from providing individualized nutritional advice, to support in managing chronic diseases, such as diabetes, weight management, heart disease, food allergies, kidney disease, digestive issues, and more.
Laurynn received her bachelor’s degree in Dietetics from Iowa State University where she also completed her dietetic internship and Master’s degree. She has a special interest in Diabetes Education, Sports Nutrition, and Eating Disorders. You can usually find her baking, reading a good book, or cuddled up with her dog.
