This is a Full-Time position working Mon,Tues,Thurs, and Fri 7am-5pm.
Essential Functions
- Work closely with medical provider to help ensure that patients have comprehensive and coordinated care. Follow-up with patients should be continuous from initial identification through closure.
- Collaborate with other professionals to assess client needs and social determinates of health and/or plan treatments.
- Act as patient advocate and liaison between the patient/family and community service agencies (i.e. schools, Department of Human Services, hospitals, support groups, etc.)
- Responsible for establishing trusting relationships with patients and their families while providing general support and encouragement
- Advocate for clients or patients to resolve crises.
- Intervene in crisis situations to assist clients.
- Refer patient, client, or family to community resources to assist in recovery from mental or physical illness and to provide access to services such as financial assistance, legal aid, housing, job placement or education.
- Follow-up with patients via phone calls, home visits, and visits to other settings where patients can be found.
- Refer individuals to educational or work programs.
- Refer clients to community or social service programs.
- Conduct Medicare/Medicaid eligibility determination and assist with completion of applications and enrollment.
- Complete documentation required by programs or regulations
- Develop or advise on social policy and assist in community development.
- Advise others on social or educational issues
- Identify environmental impediments to client or patient progress through interviews and review of patient records.
- Collect information about clients.
- Interview clients to gather information about their backgrounds, needs, or progress.
- Evaluate potential problems in home or work environments of clients
- Work independently with minimal supervision.
Job Requirements
- Bachelor’s degree in, Public Health, or Social Work is preferred.
- 2-3 years of experience in areas related to public health, social work or patient advocacy. Experience in a medical environment a plus.
- Must possess a cheerful and extroverted personality that reflects a positive attitude toward the hospital and community. Appearance is always neat and well-groomed. Works effectively with others.
- Must demonstrate proficiency with MS Office and Excel and be able to interpret data into meaningful information.
- Must possess strong organizational skills, be detail oriented, and able to interact and build relationships both internal and external.
- Must have current Mandatory Reporter training for Adult and Child Abuse.
- BLS Certification
- Other duties as assigned.
